The Care Coordinator position works under the direct supervision of the Regional Manager with general supervision from the Director of Client Services. Care Coordinators are responsible for locating, coordinating, and monitoring services and resources for all individuals over the age of 60, and individuals age 21 through 64 with a physical disability, to include, minorities, low-income and rural senior citizens, as needed. Provides support, information, assistance, and education to individuals which are providing care to elderly family members.AGENCY EXPECTATION OF EMPLOYEE

Respond to referrals to assist all individuals over the age of 60 and/or with disabilities who need care coordination, to include all minorities, low-income and rural senior citizens. Assisting above identified clients with programs of which they meet eligibility, including ARChoices, Targeted Case Management, State Case Management, MIPPA, SHIIP, Caregiver and VA Conduct needs assessments and develops service plans Coordinate and monitor services and client situation Complete required documentation and reporting Provide information, referral services and application assistance on a variety of programs and benefits to clients, families, and caregivers Advocate for the rights and needs of the senior citizen population Advocate for clients & assist them with gaining access to medical, social, educational, and other services as appropriate Assist in identifying and documenting service gaps in designated service area Conduct outreach in designated service area to inform all individuals over the age of 60 of available services and resources Coordinate with other AAA programs, service providers, and agencies providing services to individuals in the target population Complete required documentation and reporting in a timely manner Attend training as provided or required Conduct Medicare Counselling in partnership with the SHIIP program Install, monitor/troubleshoot, and maintain/replace Personal Emergency Response Systems (PERS) Assist in the marketing, development and growth of the Private Pay and Durable/Non-durable Goods Programs Provide comprehensive and accurate Information and Assistance to clients, family of clients, caregivers, staff, and other agencies. Attend Interagency Meetings in your service area as assigned Assists in ensuring that resource files and resource directory are updated Duties and responsibilities may be added, deleted, or changed at any time at the discretion of management, formally or informally, either verbally or in writing.JOB CLASSIFICATION
Non-Exempt Basic Qualifications Licensed in the state of Arkansas as a social worker (licensed master social worker or licensed certified social worker), a registered nurse or a licensed practical nurse; or Have a bachelor's degree from an accredited institution in a health and human services or related field; or Have two years' experience in the delivery of human services, including without limitation having performed satisfactorily as a case manager for the period of two (2) years. Experience must be within the past three (3) years. Ability to communicate with clients, other employees, supervisors and other contacts verbally and/or in writing. Ability to understand and follow verbal and written instructions, guidelines, policies and procedures. Ability to do multi-tasking with minimal errors. Ability to travel both short and long distances. Ability to work with minimal supervision.
