You'll collaborate closely with primary care providers, specialists, and hospitals to build and support individualized care plans for patients with chronic conditions. Join our growing team as a Care Team Coordinator and play a vital role in improving patient outcomes. About the Role In this position, you will help remove barriers that prevent patients from receiving timely care—ultimately helping reduce unnecessary emergency department visits. You'll collaborate closely with primary care providers, specialists, and hospitals to build and support individualized care plans for patients with chronic conditions. K ey Responsibilities Provide at least 20 minutes of monthly care coordination for Medicare patients Conduct telephone outreach and follow-up with patients Document patient interactions accurately in the medical record Develop and update individualized care plans Perform chart reviews and track patient progress Coordinate care between providers, specialists, and facilities Educate patients on their conditions and treatment plans Collaborate with the care team to evaluate outcomes and interventions Qualifications Certification as a Medical Assistant or active LPN license Strong computer skills (Microsoft Windows, typing, system navigation) Ability to multitask efficiently between calls and documentation Excellent verbal and written communication skills Strong interpersonal skills with patients and healthcare professionals Self-motivated with strong organizational and patient assessment abilities Willingness to learn and adapt in a fast-paced environment Join us in delivering compassionate care through effective coordination!
