Job Description:
Key Responsibilities and Accountabilities of this Role Develops, documents, evaluates, monitors, and modifies the Person-Centered Service Plan (PCSP) for assigned members, ensuring it includes member choices and covers areas such as Behavioral Health, Community Resources, Crisis Safety, IDD/Waiver, Financial Information, Mental Health, Physical Health, and Substance Abuse. Ensures that members' health, safety, and service needs are met by working directly with care coordination team(s), individual members, their families/guardians, their providers, and other members of their circle of support. Maintains regular contact with care coordination team(s), members, their families/guardians, providers, community resources, stakeholders, and other professionals. Conducts in-person visits at least once every quarter, with more frequent monthly visits for complex cases, and maintains contact via telephone and virtual meetings. Provides support on cases that are not assigned to the caseload, ensuring continuity of care and seamless transitions. Provides information to care coordination team(s), members, families/guardians, providers, and other parties as required to implement PCSP. Escalates risks and barriers to appropriate team member(s) and documents all discharge planning efforts in members' records. Hosts rounds call 2 to 3 times per week with traditional care coordination teams, DCFS clinical facilitators, Elevance partners, and various other internal and external stakeholders to ensure comprehensive and coordinated care for members. Participate in general discussions and meetings regarding members, ensuring all relevant parties are informed and involved in the care process. Develops educational materials, processes, workflows, etc., to improve efficiency and outcomes, reduce risk, and reduce escalation. Provides these materials to care team members. Coordinates and schedules appointments, meetings, and travel across the entire state of Arkansas. Manages one's own electronic calendar utilizing Microsoft Outlook and ensures timely and efficient travel arrangements. Maintains compliance with Conflict-Free Case Management rules, as well as all applicable rules and regulations within the Arkansas PASSE Medicaid Provider Manual, Section II, 240.000, the Arkansas PASSE Care Coordination Agreement, Arkansas Insurance Department, Arkansas Medicaid, and other applicable Medicaid Manuals and updates. Always maintains confidentiality and integrity of Arkansas Provider Coalition, complying with The Health Insurance Portability and Accountability Act (HIPAA) and Arkansas Provider Coalition policies. Operates a company-issued laptop computer with Microsoft Office/Microsoft 365 suite products, company-issued smart phone, and other communication and database software in the performance of essential job functions. Operates a personal automobile in the performance of essential job functions for travel to meetings, members' homes, provider offices, conferences, and training. Maintains a home office with reliable internet that supports standard home office equipment, free from common distractions, and ensures the security of company-issued assets and Protected Health Information (PHI). Tasks may be performed in various locations, including the home office, member homes, provider offices, APC corporate office, car, and public areas. Minimum:
Completion of a bachelor's degree in social work, psychology, sociology, another social science, or a health-related field.Preferred:
Completion of a master's degree in social work, counseling, psychology, another social science, or a health-related field.Experience:
Minimum Requirements:
2 years of experience in coordinating care. One year of beginner-level experience withMicrosoft Excel, Outlook, and Word Preferred Qualifications:
3-5 years of experience in care coordination within managed care and/or the Arkansas PASSE (Provider-Led Arkansas Shared Savings Entity) is highly preferred.Preferred licensure/designations:
Licensed Clinical Social Worker (LCSW), Licensed Master Social Worker (LMSW), Licensed Professional Counselor, Licensed Practical Nurse (LPN), Qualified Behavioral Health Provider (QBHP).Additional Knowledge, Skills, and Abilities:
Knowledge of behavioral support strategies and trauma informed care. Knowledge of medication techniques and meeting facilitation strategies. Knowledge of computer fundamentals and business software, including database, communication, email, presentation, spreadsheet, and word-processing software. Knowledge of available community and state resources. Skill in presenting effectively while conveying information to multiple parties. Skill in the operation of a computer, utilizing database, communication, email, presentation, spreadsheet, and word-processing software. Ability to complete cultural competency training. Ability to mediate meetings between multiple parties, ensuring effective communication, conflict resolution, and collaborative decision-making. Ability to enter member information into a computer utilizing an electronic medical record (EMR) system. Ability to communicate effectively in English, both orally, with individuals and groups, and in writing. Ability to communicate effectively with a diverse group of individuals, various ages, ethnicities, and social and economic backgrounds, in the provision of services. Mandatory Expectation 1. Must possess a valid Arkansas Class D (Non-Commercial Vehicle) Driver's License before employment and maintain licensure for the duration of employment in this position. 2. Must have access to a properly licensed and insured, reliable automobile for reimbursable assigned business travel. 3. Must provide proof of current, valid automobile insurance before employment in this position and thereafter upon request; must maintain current, valid automobile insurance for the duration of employment in this position. 4. Must reliably commute to/from member homes, provider offices, and meeting spaces as needed. 5. Cannot be excluded or debarred under any state or federal law, regulation, or rule, or ineligible or prohibited to enroll as a Medicaid provider. 6. The incumbent will be required to stand and/or sit for long periods of time. Some essential functions of this position are typically performed in an environment with a quiet or moderate noise level. Some exposure to a loud noise level will be required. The incumbent may be exposed to paper dust, printer ink/toner, and common office chemicals. The incumbent will be exposed to typical outdoor weather conditions including cold, hot, high humidity, and rainy weather, when traveling to different locations, and to animals/pests.
