  • Full-time Job Summary We are seeking a dedicated and compassionate Client Care Coordinator to join our dynamic healthcare team.
In this vital role, you will serve as the primary point of contact for patients and their families, ensuring seamless communication and coordination of care. Your expertise will help facilitate discharge planning, intake processes, and case management, all while maintaining compliance with healthcare regulations. This position offers an exciting opportunity to make a meaningful difference in patient experiences by providing exceptional service and support within a fast-paced hospital environment. Duties Act as the main liaison between patients, families, healthcare providers, and insurance companies to coordinate care efficiently. Manage patient intake procedures, including verifying insurance coverage and collecting necessary documentation. Assist with discharge planning by coordinating services, scheduling follow-up appointments, and ensuring smooth transitions of care. Utilize Electronic Health Record (EHR) and Electronic Medical Record (EMR) systems to document patient interactions accurately and promptly. Conduct utilization review and management to ensure appropriate level of care while adhering to HIPAA regulations for patient privacy. Support case management activities by assessing patient needs, developing care plans, and collaborating with multidisciplinary teams such as social work, behavioral health, and nursing staff. Maintain comprehensive knowledge of medical terminology, CPT coding, ICD-9/10 coding systems, and hospital-specific protocols to facilitate accurate documentation and billing. Requirements Proven experience in critical care settings such as ICU, emergency medicine, or trauma centers is highly preferred. Strong background in hospital environments with familiarity in acute care, hospice care, or pediatric units like PICU. Knowledge of medical coding systems including
CPT, ICD-9/10, ICD
coding for accurate billing and documentation. Experience working with EMR/EHR systems and understanding of HIPAA compliance standards. Excellent communication skills with the ability to handle sensitive situations professionally and empathetically. Ability to multitask effectively in a fast-paced hospital setting while maintaining attention to detail. Prior experience in discharge planning, utilization review/management, or case management is desirable. Certifications or experience in social work or behavioral health are a plus.
