This role plays a vital part in supporting patients with multiple chronic conditions by ensuring continuity of care and improved health outcomes. Chronic Care Management (CCM) is a Medicare-supported program designed to provide ongoing care and support for patients with two or more chronic conditions. The goal is to help manage their health more effectively, prevent complications, and improve quality of life through personalized care and coordination Responsibilities Develop personalized care plans in collaboration with patients, caregivers, and healthcare providers. Coordinate and facilitate communication between healthcare team members, including physicians, specialists, nurses, and ancillary services. Monitor patient adherence to treatment plans and medications; support progress toward health goals. Provide ongoing education and support to patients and families regarding disease management, lifestyle changes, and preventative care. Utilize Electronic Health Records (EHR) to document encounters, track interventions, and ensure continuity of care. Collaborate with community and social service agencies to address social determinants of health and support overall well-being. Conduct regular follow-up calls and care coordination activities to improve outcomes and quality of life for chronic care patients. Qualifications Minimum 1 year of clinical experience as a Medical Assistant, CMA, or LPN. At least 1 year of experience working with Electronic Medical Records (eClinicalWorks preferred). Strong verbal and written communication skills in both English and Spanish. High level of professionalism and strong customer service skills. Completion of a Medical Assistant program, CMA, or LPN certification required.
