Every day, you'll oversee and coordinate comprehensive care coordination initiatives, establishing protocols, driving process improvements, and managing resources to enhance patient care journeys and operational efficiency. You'll lead, mentor, and empower your team, fostering excellence, collaboration, and continuous improvement, while upholding quality, safety, and compliance. To be successful, you'll combine strong clinical acumen with exceptional organizational and analytical skills, a proactive problem-solving approach, and the ability to inspire your team, transforming strategic visions into tangible, positive impacts. Provides direct daily supervision and leadership to the care coordination department, including staff supervision, coaching, counseling and performance management Reinforces care coordination programs and strategies to optimize patient care, resource utilization, and quality outcomes that align with system-wide Care Coordination Ensures adherence to all relevant healthcare regulations, accreditation standards, and organizational policies Assists with the daily operations of care coordination Collaborates with internal and external stakeholders to coordinate patient care and facilitate communication across the care continuum Dignity Health now offers an Education Benefit program for benefit-eligible employees after 180 days.
