Are you personable and have excellent observation, verbal and written communication skills to individualize plan of care with elderly disabled members? This may just be for you! At Upscale Home Care, we are looking for a Lead Care Coordinator who is responsible for planning, implementing, and coordinating all case management activities with members to evaluate the medical needs and plan of care in order to facilitate the member's overall wellness. We want to ensure that care services are delivered appropriately and for the supervision of home care personnel.
Responsibilities include:
Going down to the members home for initial orientation and monthly check ins. Engage with the members. Offering companionship and emotional support. Development of individualized care plan. Monitor and document patient's condition.
Must have:
Excellent communication skills and ability to sympathize with and understand member's needs. Valid driver's license. Ensure compliance with HIPAA regulations while maintaining patient confidentiality.
