Our company services seniors in the Greater Northern/Western Phoenix Area, including Sun City, Surprise, Peoria, Phoenix, Anthem, North Scottsdale. We are not looking for applicants who reside in the East Valley - Mesa, Chandler, Gilbert.

A Care Manager plays a vital role in supporting clients and their families as they navigate the complexities of aging with grace and dignity. Acting as the eyes and ears for family members, the Care Manager ensures that clients' needs are met with compassion and professionalism while providing peace of mind. Guided by the question, "What would I do if this were my parent?" , Care Managers treat each client as they would their own family member. They build trust through regular check-ins, open communication, and coordinated care while fostering collaboration with healthcare professionals to enhance client well-being.
Position Summary:
As an Elder Care Manager at AZCMS, you will assess client needs, coordinate resources, and advocate for individuals navigating the challenges of aging. Your role involves case load management, care plan development, thorough documentation, collaboration with professionals, and crisis intervention, all with a client-first approach rooted in dignity and respect.
Key Responsibilities:
Case Load Management Prioritize and manage tasks based on client needs and urgency. Schedule and monitor appointments and visits efficiently. Track progress and promptly address emerging issues to ensure high-quality care. Care Plan Development and Implementation Conduct comprehensive assessments of client health, medical history, and social needs to build tailored profiles. Develop and implement care plans to address current and future concerns, enhancing client well-being and peace of mind. Regularly monitor progress and adjust plans based on evolving needs. Thorough Documentation and Attention to Detail Maintain detailed progress notes that are timely, accurate, and professional. Clearly document next steps or follow-up actions for seamless care coordination. Adhere to confidentiality standards and ensure information supports effective communication among team members. Collaboration with Families and Professionals Foster open communication with clients, families, and responsible parties to align on care goals. Collaborate with healthcare providers and social networks to deliver comprehensive support. Share critical information to ensure continuity of care across all aspects of health. Advocacy and Crisis Intervention Identify and address client needs during crises, including safety concerns or medical emergencies. Provide emotional support and empower informed decision-making for clients and families. Collaborate with professionals to coordinate effective interventions and ensure follow-up care.
Qualifications:
Education:
Associate's degree required; Bachelor's preferred in Social Work, Nursing, or a related field.
Experience:
2+ years in healthcare or elder care. Experience in care planning or social work is a plus.
Skills:
Proficiency in Microsoft Office 365. Strong communication, organization, and interpersonal abilities.
Other Requirements:
Reliable vehicle to transport clients and valid driver's license. Willingness to travel and do home visits.
Personal Qualities:
Compassionate, detail-oriented, and passionate about supporting seniors.
