If you're driven, collaborative, and energized by making a real difference— this is where you belong . _________________________________________________ About the Role The Intake Specialist serves as a critical first point of contact for members entering New Freedom's Intensive Outpatient Program. This role ensures a smooth, compliant, and supportive intake experience by conducting assessments, facilitating onboarding processes, and setting the foundation for successful treatment engagement. You will partner closely with clinical and cross-functional teams to ensure members understand program expectations, their rights, and their individualized treatment pathway from day one. This role is ideal for someone who: Thrives in a fast-paced, mission-driven environment Is highly organized and detail-oriented Has strong interpersonal and assessment skills Is passionate about behavioral health and helping individuals succeed Can balance empathy with accountability _________________________________________________ What You'll Be Responsible For Conduct comprehensive intake assessments and document appropriate diagnoses Complete individualized treatment plans aligned with member goals Facilitate all intake paperwork, consents, and admission documentation Educate members on program expectations, policies, rights, and responsibilities Ensure compliance with confidentiality, regulatory, and program requirements Provide orientation to all new admits within required timeframes Collaborate with clinical staff to ensure seamless member transitions into care Communicate program details including services, schedules, grievance processes, and expectations Support member understanding of treatment compliance and available resources (including dual diagnosis services) Maintain accurate records within electronic health record systems Meet established KPIs focused on member care, engagement, and intake timeliness Participate in ongoing clinical supervision and required continuing education _________________________________________________ What Success Looks Like Members experience a clear, welcoming, and well-organized intake process Intake documentation is accurate, compliant, and completed on time Strong collaboration with clinical teams ensures seamless handoffs Members demonstrate understanding of program expectations and engagement in treatment Consistent achievement of intake KPIs and quality standards Positive feedback from members and internal teams _________________________________________________ What We're Looking For Master's degree in Social Work or related field Minimum of 4 years of experience in behavioral health, including intake assessments and treatment planning Strong communication, organizational, and critical thinking skills Experience with motivational interviewing and client engagement Ability to work both independently and as part of a team Experience working with dual diagnosis populations (preferred) Ability to adapt in a fast-paced, evolving environment (preferred) Strong commitment to member-centered care (preferred) Must be 21 years of age or older Ability to pass background check, drug screening, and TB testing _________________________________________________ Why You'll Love Working Here We take care of the people who take care of others.
