Job Description

Every day you will support safe patient care through multidisciplinary processes and coordinating unit activities. You will efficiently manage information flow and assist healthcare team members in delivering care across all patient age groups.
To be successful in this role, you will demonstrate strong organizational and communication skills, a proactive approach to managing day-to-day operations, and a commitment to providing safe, therapeutic care while working effectively with the entire healthcare team. Collaborates with and supports the Care Coordination team, including, but not limited to the RNCC and Social Worker. Manage timely post-acute care referrals, to assist with length of stay management and mitigation of denials. Communicate and collaborate with interdisciplinary teams, patients, and family to facilitate care progression. Ensure post-acute follow-up appointments for identified patients to assist with readmission prevention and ensuring continuity of care. Deliver required patient notifications and letters, and obtain patient or family signatures on required documentation as needed to meet regulatory compliance. Manage transportation referrals as requested.

Job Requirements Required:
Associates Other Associate (two-year) Degree in Business/Health-Related field, upon hire or High School Graduate General Studies, upon hire or High School GED General Studies, upon hirePreferred:
At least 3 years previous experience in health care, care coordination, utilization review, discharge planning or similar healthcare setting. and Bachelors Other or BSW, upon hire Experience supporting clinicians in demanding and fast paced patient care environmentsLicensed Practical Nurse:
