Job Description

Lead patient care coordination and behavioral health operations as a Care Coordinator Supervisor , while making a meaningful impact in a collaborative hospital setting, a few hours outside of Sedona, AZ.
Salary:
$101,000 - $152,000
Annually Care Coordinator Supervisor Opportunity:
$5,000 Sign-On Bonus | Campus Housing Available | Full-Time | Permanent Position | Monday-Friday Schedule | four 10's or five 8's The Care Coordinator Supervisor will manage patient referrals and transfers to outside hospitals, ensuring continuity of care and timely placement Provide clinical leadership while supporting behavioral health and interdisciplinary care teams The Care Coordinator Supervisor will coordinate with physicians, case managers, and healthcare professionals to optimize patient outcomes Join a mission-driven hospital offering leadership opportunities, professional growth, and a collaborative work environment
Care Coordinator Supervisor Qualifications:
Active Licensed Professional Counselor Supervisor (LPC-S) license or Registered Nurse (RN) license required Preferred that a LPC-S/Registered Nurse has a Bachelor of Science in Nursing (BSN) A Care Coordinator Supervisor could have previous leadership, care coordination, or supervisory experience preferred Excellent communication, critical thinking, and interdisciplinary collaboration skills
