Care Coordinator- Life in Balance Pascua Yaqui Tribe - 3.8 Tucson, AZ Job Details Full-time $23.36 an hour 1 day ago Qualifications CPR Certification BLS Certification Driver's License First Aid Certification Driving Full Job Description JOB The Care Coordinator is a senior care specialist (BHT) who helps lead other care specialists in addition to providing case management, assessments, including treatment recommendations, placement coordination and client guidance. The Care Coordinator assists licensed Behavioral Health Clinicians in the delivery of client services and helps train BHT1 and BHT2 staff.
EXAMPLE OF DUTIES
Perform case management activities, such as assessing patient eligibility for benefits programs, consulting with other departments and agencies to coordinate care for patients, and participate in staff meetings to determine best treatment for patientsConducts educational programs; monitor topics and provide input into structure of group. Conduct groups when a supervisor is not present.

Conduct comprehensive assessments and complete intake documentation.

Screen and enroll clients in the behavioral health system or Sewa Uusim programs and/or TRBHA programs.

Responsible for coordinating contract services.

Track all residential placements for clients.

Maintain and update electronic client records and files; document important information in charts and assist in charting client progress.

Monitor and document client behavior; ensure compliance with rules and regulations.

Record all clinical contacts and complete all appropriate Arizona State Enrollment forms, treatment/service plans, comprehensive assessments, updates, and closures.

Provide information to clients regarding client rights and ensure their rights are protected.

Conduct outreach activities and provides information about programs and group therapy.

Participate on Treatment Team Review Meetings (TTR), engage in Clinical Supervisions, and present at TMDR for contracted services as needed. Transport clients to scheduled program activities including support groups, community events, and vocational recreational activities.

Participate in community events and prevention activities as required by Program Manager.

Perform other duties of a similar nature or level as requested by supervisor or director.

Specialized functions for Sewa U'usim may include: Responsible for setting up and conducting family Wraparound Meetings.

Coordinate Natural support referrals to Alternative Medicine, Tribal Healers, other programs.

Responsible for conducting Life Skills groups with Youth and Young Adults.

Review Treatment Plans and write observational notes, attend staffing, and provided specialized clinical support services as directed.

Conduct Special Summer/Winter and School Break activitiesDevelop and write curricula for therapeutic groups.
SUPPLEMENTAL INFORMATION
Bachelor's Degree in Behavioral Health or four (4) years of experience in a behavioral health setting; or an equivalent combination of education and experience sufficient to successfully perform the essential duties of the job such as those listed above.

ANDBilingual in English/Spanish preferred but not required.
Special Requirements:
CPR and First Aid Certifications or obtain within ninety (90) days of hire. Failure to maintain a current CPR and First Aid Certification may result in termination.

Employment at the Yoeme Kari Group Home or Men's Path requires incumbents to be at least 21 years old.

Must possess and maintain a valid Arizona Driver License.

This position may be required to work non-traditional hours, nights, and weekends.

Must have a current Level 1 Arizona Clearance Card. Failure to maintain a current Level 1 Clearance Card will result in termination
