We currently operate multiple healthcare sites in Costa Mesa, Santa Ana, Mission Viejo and Newport Beach We currently have 2 full time Bilingual Care Manager openings: Costa Mesa Clinic - Care Manager Mission Viejo Clinic - Care Manager We are seeking a dedicated and compassionate Care Manager to join our healthcare team. This role offers an opportunity to make a meaningful difference in patient lives by providing expert guidance and support throughout their healthcare journey.
Summary:
The Care Manager provides Care Management services to at risk patients with multiple medical and psychosocial needs by developing individualized treatment plans, coordinating social services, performing outreach, follow-up and working as an effective member of the treatment team. The Care Manager works in collaboration with Share Ourselves service teams, managed care plans, and partner service providers to develop and implement treatment/population-based coordination that aims to improve the quality of life and health outcomes of Share Ourselves patients and patients. With a primary focus on advancing Share Ourselves integrated model of care in addressing Social Determinants of Health (SDOH), the duties of the position include fostering the care coordination delivery system, promoting intervention protocol principles, monitoring outcomes, cultivating network partnerships, and advocating for the care of the patient Duties Manages the care of patients in collaboration with other health professionals and members of the health care team in order to make decisions about treatment and attain health goals. Completes comprehensive assessments and ongoing re-assessment of the patients to evaluate bio-psycho-social needs. Based on assessment information develops and initiates the plan of services, which is patients -centered and consistent with program guidelines and policies and procedures. Provides culturally responsive services to support patients engagement and understands how trauma and other psychosocial factors might influence patients' participation in social services and care. Actively assesses and responds to a variety of situations that may include crisis intervention and conflict resolution. Establishes and maintains a care management relationship with patients and their informal support network as appropriate, offering respect, dignity and support. Provides equitable and culturally responsive crisis intervention, advocacy, problem solving, and other services. Documents all care management activity regarding identified problems within required time frames, adding any new problems to the plan of services, as needed. Maintains required written documentation and follows a clear, concise, and consistent system of charting to allow for continuity of care. Maintains patient files according to company policy and contract requirements. Establishes and maintains open and effective communication with community providers, including physicians and other health care and social service workers. Provides appropriate information on all significant aspects of individual patient care and program operations, while maintaining necessary confidentiality. Utilizes manager to address challenges and escalate open issues while maintaining open lines of communication and providing updates on caseload activity. Comes prepared to team and/or manager supervision meetings. Will coordinate with providers and support teams clinically appropriate treatment modalities such as, Harm Reduction, Motivational Interviewing, and Stages of Change in all interactions with patients, including assessments and case management to maintain fidelity and program standards. Provides ongoing supportive services to enrolled patients in accordance with Share Ourselves Care Management guidelines. Assists patient in accessing Federal and State program offerings with benefits establishments (SSI, Veterans pension, etc.), obtaining ID, birth certificate, social security cards, provide employment linkages and assistance, housing referrals, and health related assistance. Supports individualized case management services including crisis intervention, assistance in accessing public benefits, information and referral regarding access to health, social services, and encourages participation in educational and self-help supportive Attends appointments with patients or may liaison with patient in care/treatment facility which requires Care Manager to be able to drive to/from various locations. (Does not drive patients in personal vehicle) Follow-up on EHR in-basket message and management tasks EDUCATION and / or
EXPERIENCE
Degree from an accredited college in social work, human services or a related field; a combination of education and experience may be substituted. Minimum 2 years Care Management/Case Management experience in working with culturally diverse populations, including substance abuse, mental health, LGBTQ+, HIV/AIDS, co-occurring, low-income, and those experiencing homelessness. Experience working with individuals who are living with complex mental illness, substance addiction and physical health problems. Experience cultivating and maintaining productive, professional relationships with various stakeholders required Experience working and documenting within an EMR/EHR system, Epic preferred. QUALIFICATIONS
Demonstrated knowledge of Care Coordination and/or Case Management methodologies. Strong verbal and written communication skills. Empathetic, self-motivated, problem solver and flexible work methods Understanding of the importance of quality and timely data collection and reporting Excellent professional boundaries Ability to work under minimal supervision both independently and within a multi-disciplinary team in various work environments both on-site and off-site. Ability to prioritize tasks and be detail oriented. Skilled in organizing and maintaining accurate charts/records. Good time management skills Bi-lingual English and Spanish required Must have valid drivers license w/o restriction and maintain CA state vehicle insurance at all times. What do we offer?
