Job Description:
Complete Care Community Health Center (CCCHC) is a growing Los Angeles County Health Care Center with Clinic locations throughout the area. We are seeking an experienced care coordinator or Patient Care Coordinator which is responsible for scheduling, organizing, and managing all aspects of a patient's healthcare maintenance and treatment once a specialist report has been received from care referred out. Their duties include communicating with the internal healthcare team about a patient's needed continuing care and educating patients about resources and options for managing their health helping to close the loop of the referral and care not only for the organization but also for the patient. The successful candidate will have a willingness to work closely with patients, physicians, and other member of the health care team. Must be professional, detail-oriented, have exceptional phone skills and be able to handle a very fast-paced environment.
Principal Duties and Responsibilities:
Initiate outreach and missed appointment procedures, as necessary Document client services in medical records Establish and retain client referral systems from care coordination systems Answers telephone in pleasant manner and deals with customer needs expeditiously. Responds to email and text inquiries from internal and external sources. Provides any necessary and related instructions/directions. Updates patient information as necessary. Maintains patient confidentiality with all patient information. Escalates issues to supervisor for resolution, as deemed necessary. Perform all other duties and tasks assigned by the supervisor or manager.
Qualifications:
EHR experience, NextGen (Preferred) 2 years minimum of case management experience Must be familiar with medical terminology Bi-lingual (Spanish/English) Must have strong communication skills, both verbally and in writing Must be organized Must have strong computer skills Must be able to handle a high call volume; call center experience is a plus Must have the ability to multitask Must be a hard-working and self sufficient Have the ability to problem solve Demonstrate Empathy with callers
