Inglewood, CA Job Details Full-time $30 - $35 an hour 1 hour ago Benefits Health insurance Dental insurance Paid time off Vision insurance Professional development assistance Life insurance Retirement plan Qualifications Teamwork Care coordination Interdisciplinary behavioral health coordination Full Job Description Coordinate care with medical providers, mental health professionals, probation officers, social workers, and community agencies Assist clients with referrals for housing, employment, healthcare, transportation, education, and recovery support services Maintain accurate and timely documentation, case notes, progress reports, and discharge summaries Advocate for clients and assist with navigating healthcare and social service systems Develop individualized treatment plans and recovery goals in collaboration with clients Provide individual, group, and family counseling sessions Monitor client progress and update treatment plans as needed Work collaboratively with clients, treatment providers, community resources, and support systems to ensure continuity of care and successful treatment outcomes.
