Job Description

We are looking for a Clinical Case Manager to join our Home Health Agency. This role will work closely with our intake and clinical team and collaborate directly with our current Clinical Coordinator to support patient care and daily clinical operations. The ideal candidate is organized, proactive, and experienced in Home Health coordination, ensuring smooth communication between physicians, field staff, patients and office team.
Key Responsibilities:
Coordinate patient care in collaboration with intake, clinicians and clinical coordinator Evaluates hospitalized patients referred for home health. Communicate with Physicians, patients, families, and staff regarding care updates Assist with scheduling and coordination of patient visits Follow-up on pending orders, authorizations, and documentation Support care plan updates and ensure timely completion of clinical paperwork Monitor patient census and assist with care coordination needs Maintain accurate and compliant documentation in EMR (Axxess or similar) Provide Overall support to ensure continuity and quality of patient care Completes referral information that includes intake data, essential background information, hospital course, and the plan of care. Maintains liaison relationship with hospital and insurance personnel, providing information and education on Organization services, coverage issues and related areas. Works with Clinical Supervisors and staff clinicians to gather, collate, and report patient care summary information to patient care provider and/or managed care
Case Managers Minimum Qualifications:
Active CA LVN or RN License (Required) Minimum of one year home health experience Knowledge of home health regulations and documentation standards Case Management experience Experience with EMR systems Strong organizational, multitasking, and communication skills Ability to work in a fast-paced team environment Reliable, detail oriented, and proactive with follow-up Employment type Full-Time (In-Office) Woodland Hills, CA Please submit your most up-to-date resume
