Job Description

Key Responsibilities:
Intake Coordination:
Receive and process new patient referrals from hospitals, physicians, and other sources. Gather and verify patient information, including medical history, insurance, and service requirements. Communicate with patients and families to explain services, address inquiries, and set expectations. Coordinate with clinical staff to assess patient eligibility and care needs. Maintain accurate and organized records in compliance with HIPAA and agency protocols.
Staffing Coordination:
Schedule and assign appropriate home health staff to meet patient care needs, considering skill set, location, and availability. Monitor and adjust schedules to address last-minute changes, call-offs, or emergencies. Communicate staffing assignments and updates to caregivers promptly. Ensure staff compliance with certifications, licenses, and training requirements.
Administrative Support:
Collaborate with the clinical team to ensure proper documentation and reporting. Utilize agency software to manage patient and staff data effectively. Assist in tracking and reporting performance metrics, such as patient satisfaction and staff efficiency. Participate in team meetings to discuss patient care plans and staffing needs.
Customer Service:
Serve as a liaison between patients, families, and healthcare providers to ensure clear communication and satisfaction. Address and resolve patient or staff concerns professionally and promptly.
