We are looking for a Licensed Vocational Nurse( Care coordinator) Job Functions / Responsibilities Plans, Organizes and Directs home care services Completes an initial assessment of patients to determine home care needs Regularly re-evaluates the patient's nursing needs. Initiates a plan of care and makes necessary revisions as patient status changes Counsels the patient and family in meeting nursing and related needs Provides health care instructions to the patient as appropriate per assessment and plan of care Communicates with the physician regarding the patient's needs; obtains/receives physician's orders as required Who We Are Hollywood Home Health Services is an industry-leading skilled nursing agency based in Southern California.
