We are a Medicare-certified home health agency in Sherman Oaks, and we are looking for an experienced LVN to join our clinical team as a Care Coordinator. If you have at least one year of home health care coordination experience — managing active patient caseloads, tracking visit compliance, and keeping episodes on track from start of care through discharge — we would love to hear from you. THE ROLE LVN
•Home Health Care Coordinator In-Office | Sherman Oaks | Monday-Friday | No field visits. No weekends. No on-call. Once a patient is admitted, you own their episode. You will be the person keeping everything on track — from start of care through recertification and all the way to discharge. You work closely with our Director of Nursing and field clinicians to make sure no patient falls through the cracks and every episode is compliant, documented, and moving forward. Here is what your days will look like: Managing an active caseload of home health patients across the full episode of care Monitoring visit frequency and compliance against the plan of care — catching shortfalls before they become problems Tracking recertification timelines and initiating the recertification process ahead of deadlines — no surprises Reviewing clinical documentation in WellSky (Kinnser) for completeness and flagging deficiencies to field staff and the DON Coordinating physician order follow-up — verbal orders, plan of care updates, and outstanding signed 485s Communicating with field clinicians on open items, care plan changes, and documentation corrections Escalating clinical concerns, missed visits, and documentation gaps to the DON in real time Supporting QA and compliance activities including chart reviews and tracer preparation Participating in QAPI and contributing to agency performance improvement initiatives Assisting with discharge planning coordination and ensuring clean episode closure in WellSky You will be the person who keeps the clinical engine running between the front door and the field — and that role is critical to everything we do.•
WHAT WE'RE LOOKING FOR
Active California LVN license in good standing — required Minimum 1 year of home health care coordination experience in this exact type of role at a Medicare-certified agency — this is a firm requirement Strong working knowledge of Medicare home health Conditions of Participation, visit frequency requirements, and recertification timelines Experience reviewing clinical documentation and identifying deficiencies — you know what a compliant note looks like and what doesn't pass Familiarity with OASIS triggers — you don't complete OASIS, but you know when one needs to happen and you escalate accordingly Proficiency with WellSky (Kinnser) or a directly comparable home health EMR — you can navigate caseload management, documentation review, and reporting without hand-holding Highly organized with the ability to manage a large active caseload simultaneously without dropping anything Strong communicator — comfortable working with field nurses, the DON, and physician offices Someone who takes ownership, stays ahead of deadlines, and doesn't need to be chased•
WHAT WE OFFER
Competitive hourly rate commensurate with your experience — we will discuss numbers early so no one's time is wasted $3,000
