In this vital role, you will serve as a key point of contact for patient care coordination, ensuring seamless communication between patients, families, and healthcare providers. Your expertise in acute and post-acute care settings will drive exceptional patient outcomes, foster efficient discharge planning, and support comprehensive case management. Duties Collaborate with multidisciplinary teams to develop and implement individualized patient care plans across inpatient, outpatient, and post-acute settings. Conduct thorough patient assessments, including vital signs, medical history review, and physiological evaluations to inform care strategies. Coordinate discharge planning by arranging follow-up appointments, home health services, and community resources to ensure continuity of care. Manage patient intake processes and maintain accurate medical records in compliance with HIPAA regulations. Assist in diagnostic evaluations and monitor patient progress through ongoing assessments and documentation. Support utilization management by reviewing medical necessity criteria and facilitating appropriate resource allocation. Provide education on medical terminology, disease processes, and treatment plans to patients and their families to promote understanding and engagement. Requirements Valid LVN license with current state certification required. Critical care experience with a focus on ICU or emergency medicine strongly preferred. Experience working within Level I or Level II trauma centers or similar high-acuity environments such as PICU or burn units. Knowledge of hospital medicine, infectious disease care, behavioral health, geriatrics, and medical-surgical nursing practices. Familiarity with hospital experience including inpatient and outpatient settings; clinic experience is a plus. Strong understanding of physiology, diagnostic evaluation procedures, and patient assessment techniques. Proficiency in medical terminology, electronic medical records systems, and HIPAA compliance standards. Excellent communication skills with the ability to coordinate effectively across multidisciplinary teams. Ability to handle complex cases involving post-acute care transitions or managed care environments confidently.
