A Brief Overview The Case Management Liaison is a member of the health care team who assists in the coordination of discharge planning and insurance verification, under the direction of a Social Worker and Case Manager as assigned by the Social Work Supervisor. This position is the focal point of communication and organizing for the utilization of services and equipment, both in the acute setting and to the transitioning facility or home. Performs various clerical functions that setup a solid flow towards transitioning the patient to the next care setting. Applies the SHC - ValleyCare values to the performance of every job duty. Locations Stanford Health Care Tri-Valley What you will do Performs various clerical and secretarial functions for nursing department personnel within the assistant sphere of responsibility. Maintains master binder/file of computer generated forms, admissions, transfer packets and department logs. Assists with setting up patient transport to SNF, Home Health and hospice services. Responsible with checking UR calls, assists with faxing clinical reviews to different payors and input to EMR. Performs admission, concurrent and retrospective reviews under the direction of the Case Manager, following established guidelines. In addition, they will assist with departmental staffing time keeping input. CTI Discharge assistants will run and keep reports for CTI team discharge, track patient files, follow up on post discharge needs with SNF, and will act as liaisons for follow up appointments. Performs clerical activities and data entry related to discharge planning, utilization review, patient disposition, community referrals. Assists with compiling resource information and developing resource files. Employee must perform all duties and responsibilities in accordance with the

Education Qualifications High school diploma or GED equivalent. Required Bachelor's degree Preferred Experience Qualifications One (1) year prior experience as Discharge Planning Assistant Required Two (2) years of progressive clerical experience with referrals to HH, SNF, Transportation and DMEs Preferred Required Knowledge, Skills and Abilities Strong basic computer skills. Detail oriented. Basic knowledge of medical terminology. Knowledge of Home Health, Skilled Nursing and basic Clerical Assistant duties. Strong command of the English language. Insurance verification experience. Excellent communication skills. Licenses and Certifications BLS - Basic Life Support required . Base Pay Scale:
Generally starting at $38.12 - $49.45 per hour The salary of the finalist selected for this role will be set based on a variety of factors, including but not limited to, internal equity, experience, education, specialty and training. This pay scale is not a promise of a particular wage.
