Job Description

Job Summary We are seeking a compassionate, detail-oriented, and dynamic Resident Care Coordinator to manage all facets of resident care services within our community. The RCC acts as the key liaison between residents, families, and staff, ensuring that our care plans are followed, residents feel valued, and daily operations run smoothly. This role directly supports our Memory Care and Assisted Living areas.
Responsibilities:
In collaboration with our
Wellness Director/LVN and Executive Director Care Coordination:
Develop, implement, and update personalized care plans for residents
Staff Leadership:
Hire, train, schedule, and supervise caregivers and med-techs
Resident Health Management:
Monitor changes in resident conditions. Communicate and coordinate with families, physicians, home health and/or hospice partners.
Compliance & Safety:
Ensure compliance with all California state regulations for assisted living and memory care.
Daily Rounds:
Conduct daily rounds to ensure resident care needs are met.
Collaboration:
Work alongside Activities, Dietary, and Maintenance departments to enhance the overall resident experience. 'On the
Floor':
1or more 'on the floor' shifts per week to remain fully in touch and engaged with the nuances of our residents care, medication management, etc.
Qualifications Experience:
Prior experience as a Resident Care Coordinator (RCC) is a must . Proven background in a senior living or long-term care setting required. Memory care/dementia experience is strongly preferred.
License/Education:
High School Diploma or equivalent required. A CNA or LVN license is NOT required for this role ; we value your hands-on leadership and administrative coordination experience.
Leadership Skills:
Proven ability to mentor staff and work effectively in a team environment.
Communication:
Excellent verbal and written communication skills to correspond with families, staff, and medical professionals.
Technical Proficiency:
