Elder Care Management is currently seeking qualified full-time professionals to add to our amazing team of Aging Life Care Managers. This individual will work directly with our older adult clients in their homes and the community, providing guidance, support, oversight, and advocacy as they navigate their aging journey. Who is

Since 2010, Elder Care Management has been providing Geriatric Care Management services in the Sacramento area with a team of experienced care managers, gerontologists, and registered nurses. We are proud to be the leading Aging Life Care provider in the area supporting the greater Sacramento region, including Sacramento, Placer, El Dorado, and Yolo counties. Our care managers are proud members of the Aging Life Care Association (ALCA) and the National Academy of Certified Care Managers (NACCM), reinforcing our dedication to excellence in care management.Job Responsibilities:
Conduct regular in-home visits to evaluate client well-being and to identify potential resources to enlist to make a positive impact. Develop, formulate, and carry out detailed care plans to address opportunities to improve client's quality of life using available formal and informal resources. Meet with clients, families, and other involved professionals to communicate observations, and provide updates and recommendations. Facilitate conflict resolution and mediation for clients, families, and outside providers to ensure the well-being of clients. Provide transportation and accompany clients to medical appointments and procedures. Advocate within the healthcare setting to facilitate requests for additional testing, referrals to specialists, medication reviews, and other needs. Perform comprehensive in-home assessments and provide written and verbal summaries of findings and recommendations to responsible parties. Execute written case notes and timely documentation of billable services. Engage in ongoing education and training opportunities to enhance knowledge and skills needed to support clients, particularly within the key knowledge areas set forth by the Aging Life Care Association. Work collaboratively with an exemplary and supportive, multidisciplinary Care Management team to ensure unsurpassed client care. Perform scheduled on-call shifts, being available to support clients amidst crises and emergency room visits. What You Bring to theRole:
Mandatory Bachelor's degree in fields such as gerontology, social work, nursing, public health, health and human services, or related fields. Prefer Master's degree in related field, MSW or licensed RN. Minimum of five (5) years of experience working directly supporting older adults. In-depth knowledge of the aging process and challenges older adults often face, including dementia. Self-starter, able to work independently and in a collaborative team environment. Exceptional organizational, scheduling, and time management skills. Creative and calm problem-solver, even in stressful situations. Excellent listening, verbal, and written communication skills. Integrity, honesty, and a positive attitude with the highest standards for professional ethics. Proficiency in basic computer skills with a demonstrated ability and willingness to learn new applications and platforms. Valid California driver's license and insurance with a good driving record. Available to work Monday through Friday, 9 AM to 5 PM, with flexibility for occasional early morning or after-hours support, as well as assigned on-call evening and weekend shifts.
