Job Title:
Medicare Care Coordinator Why This Role Matters As a Medicare Care Coordinator, you'll be a trusted guide for Medicare members—helping them feel supported, understood, and confident as they navigate their healthcare. You'll build real relationships, remove barriers to care, and play a direct role in improving health outcomes. What You'll Do Be the go-to support person for Medicare members, guiding them from enrollment through ongoing care. Build meaningful relationships through proactive outreach, follow-ups, and personalized support. Coordinate care across providers and services to ensure members get the right care at the right time. Help identify and close care gaps by scheduling appointments, tracking needs, and supporting next steps. Answer questions about benefits and services with empathy, clarity, and professionalism. Work closely with internal teams and HMO/IPA partners to ensure smooth, coordinated care experiences. Support quality and performance goals by documenting interactions and following care coordination processes. Empower members by educating them on how to use their Medicare health plan and available resources. Keep accurate records to ensure continuity of care and compliance with healthcare standards. What We're Looking For 3+ years of customer service experience, ideally in healthcare, health plans, or managed care. At least 2 years of experience working with Medicare plans, benefits, or members. Understanding of HMO and/or IPA care models. Confidence juggling multiple member cases while staying organized and detail-oriented. Strong communication skills—you can explain complex topics in a simple, reassuring way. A genuine passion for helping others and delivering outstanding member experiences. Nice to Have Experience in care coordination, population health, or member engagement roles. Exposure to quality measures, care gap closure, or performance improvement initiatives. Comfort working with multiple systems and technology platforms. A collaborative mindset and ability to work effectively with clinical and administrative teams. Work Environment & Schedule This is a weekday, daytime role based in a professional corporate office setting. You'll work Monday through Friday with a consistent schedule that supports work‑life balance. The position includes standard office tools, dual monitors, and a collaborative environment where teamwork and communication are valued. Job Type & Location This is a Contract to Hire position based out of Sacramento, CA. Pay and Benefits The pay range for this position is $21.00 - $24.00/hr. Eligibility requirements apply to some benefits and may depend on your job classification and length of employment.
