Job Title:
Medicare Care Coordinator Overview The Medicare Care Coordinator serves as the primary point of contact for Medicare members, supporting them from enrollment through ongoing care. This role focuses on proactive outreach, care coordination, and delivering an excellent member experience while supporting quality initiatives and closing care gaps. The ideal candidate brings a Medical Assistant background and experience in Medicare, HMO, and IPA environments. Key Responsibilities Serve as the primary point of contact for assigned Medicare members Coordinate care from enrollment through ongoing clinical and preventative services Conduct proactive outreach to engage members and support continuity of care Identify and help close care gaps to support quality and compliance metrics Coordinate with providers, health plans, and internal teams to ensure timely care Deliver high‑quality customer service and support member questions and needs Accurately document member interactions and care activities in applicable systems Required Qualifications Medical Assistant Diploma (required) Experience working in healthcare with a focus on care coordination or patient outreach Minimum of 3 years of customer service experience Minimum of 2 years of experience working with Medicare health plans Experience working in HMO and/or IPA environments Strong communication skills and comfort with phone‑based member outreach Preferred Qualifications Experience working in IPO, HMO, or delegated medical group environments Familiarity with managed‑care workflows and member engagement initiatives Experience supporting quality initiatives or care gap closure Strong organizational skills and attention to detail Why This Role Meaningful, patient‑focused work supporting Medicare populations Collaborative, care‑centric environment Opportunity to directly impact member experience and care outcomes Job Type & Location This is a Contract position based out of Sacramento, CA. Pay and Benefits The pay range for this position is $21.00 - $24.00/hr. Eligibility requirements apply to some benefits and may depend on your job classification and length of employment.
