The Housing Solutions Supervisor, Enhanced Care Management is a direct service position responsible for overseeing care coordination of high-need members enrolled in Medical Managed Care. This position will ensure the ECM Lead Care Managers are developing a health needs assessment and care plans and providing linkages to medical, psychiatric, social, educational, and other services as needed. This position ensures the team is using best practices (i.e., motivational interviewing, trauma-informed care, and harm reduction approaches). The Housing Solutions Supervisor, Enhanced Care Management is responsible for supporting and motivating approximately 5-7 ECM Lead Care Managers. This position requires some travel to the various regions in which Mercy House operates.Mission Statement:

Client/Service Delivery Maintain a small caseload of 5 to 7 clients at any given time Engage eligible members in services and help address barriers Offer services where the member lives, seeks care, or finds most easily accessible and within CalOptima Health guidelines Accompany members to office visits as needed and according to CalOptima Health guidelines Advocate on behalf of the member with health care professionals Collaborate with area hospitals, behavioral health providers, specialists, dental providers, providers of services for Long-Term Services and Support (LTSS), and other associated entities such as Community Supports as appropriate to coordinate member care Be knowledgeable of community resources and refer members as needed Keep accurate, up-to-date case files including documentation on all clients, including but not limited to Health Needs Assessments, Care Plans, and case notes in CalOptima Connect Complete all paperwork in a timely and accurate manner that ensures payment to the agency from various sources Team Support Demonstrate leadership and team-building qualities to implement the mission, ensure quality services, and core values of the agency Train and direct new ECM Lead Care Managers and assist them with day-to-day questions relating to the completion of their job assignments Oversee quality control of case files on a quarterly basis to ensure accurate, up-to-date documentation on all clients, including but not limited to Health Needs Assessments, Care Plans, and case notes in CalOptima Provide support and guidance to ECM Care Coordinators as necessary Coordinate training and orientations as needed Meet with identified staff individually, bi-monthly or as needed Facilitate team meetings, bi-monthly or as needed Conduct 90-day probation and annual reviews for ECM Lead Care Managers Approve and monitor monthly mileage reimbursement requests Keep track of and review monthly invoices Keep up with the after-hours cellphone that is assigned to program staff and provide on-call coverage as needed Miscellaneous Assist Program Manager in overseeing the day-to-day operations for the CalAIM/ECM Program in the Manager's absence Attend staff and community meetings as assigned by the Program Manager or Housing Solutions Director Attend trainings that increase knowledge of effectively meeting clients' needs Willingness to gain knowledge and understanding of Adult Rehabilitative, Mental Health Services, and Substance Abuse Treatments Other duties as assignedRequirements :
Education and Experience Bachelor's degree in Social Work or a related field is preferred Persons with less than a BA or BS degree but with at least 2 years of direct life experience working with long-term homeless, low-income, and diverse populations and with working knowledge of mental health and addiction issues are welcome to apply Minimum of 2 years of experience in care coordination and case management for the following populations: individuals and families experiencing homelessness, individuals at risk for avoidable hospital stays (high utilizers), individuals with serious mental health and or SUD, adult nursing facility residents transitioning to the community, adults with intellectual or developmental disabilities Must possess skills related to outreach and engagement, care planning, care coordination, health promotion, transitional care support, member and family education, and social services coordination Experience working in chemical health, mental health, and co-occurring disorders is preferred Knowledge of confidentiality and the legal and ethical issues pertaining to case management Experience with HMIS data entry and CalOptima Health strongly preferred General A valid California driver's license, proof of insurance, and a reliable vehicle are required. Must be comfortable utilizing your own vehicle to conduct home visits, visit clients at shelters, meet clients in the community, etc. A flexible work schedule including some schedule changes, some weekends, and evening shifts to meet the department's needs Ability to oversee provision of ECM services and implementation of the care plan Detail-oriented and computer proficiency in Microsoft Word and Excel required Excellent leadership and team management skills Strong analytical and problem-solving abilities Excellent communication and interpersonal skills Detail-oriented with strong organizational skills Ability to handle confidential information with discretion Strong organizational and time management skills Ability to work independently and in a team environment Physical Requirements Ability to lift up to 50 pounds Prolonged periods of sitting at a desk and working on a computer Mercy House is an E-Verify employer.
