Our mission is to cultivate change that moves individuals and families from surviving to thriving through culturally responsive, trauma-informed services rooted in equity, compassion, and community. Working closely with Outreach Specialists, Case Managers, Housing Navigators, healthcare providers, managed care plans, and community partners, the Intake and Outreach Specialist ensures individuals experience a welcoming, efficient, and culturally responsive enrollment process. This position plays a vital role in identifying participant needs, removing barriers to engagement, and ensuring timely access to services. The Intake and Outreach Specialist supports program compliance by maintaining accurate documentation, tracking referrals and enrollments, monitoring timelines, and ensuring all required forms, consents, and assessments are completed in accordance with CalAIM, Medi-Cal, managed care plan, and organizational requirements. Certification Experience:
Two or more years of experience in intake, enrollment, community engagement, care coordination, social services, healthcare, housing services, or a related field Ability to establish trust and build rapport Experience coordinating with healthcare providers, managed care plans, hospitals, community organizations, public agencies, and referral partners.
