This position will assist in the coordination of resources for patients' neurologic injuries. This position will work closely with Clinical Care Managers to provide support and customer service to patients and their families.
Salary Range:
$19.58 - $28.20 per hour based on experiencePOSITION REQUIREMENTS
Licensure/Certification/Registrations:
Current driver's license required .Education:
High school diploma or equivalent required. Bachelor's Degree in a related field or related experience preferred.Experience:
Two (2) years experience in a health care setting and/or working knowledge of Social Security, Medicaid, insurance and accessing community resources strongly preferred.Skills & Abilities:
Possess strong interpersonal skills; Demonstrate the ability to effectively communicate, problem solve, and adapt to a fast-changing environment with competence. Possess a dedication to teamwork and commitment to excellence. Demonstrate a strong attention to detail. Ability to successfully handle multiple priorities and prioritize and schedule according to forecast workloads. The ability to effectively communicate with people to convey or exchange information. Ability to work competently and effectively under stress. Ability to meet frequent deadlines within short timeframes. Excellent verbal and written communication skills. Highly motivated with a positive attitude.ESSENTIAL FUNCTIONS
Assist with Social Security and Medicaid applications to include but not limited to: protective filing; completion of applications; coordination of interviews; compilation of required documentation; and persistent contact with appropriate counties to obtain Medicaid numbers. Research Medicaid and Medicaid waiver programs for patients discharging to states other than Colorado. Provide specific information pertaining to services available under these programs and the application process. Identify community resources, as requested by the Clinical Care Managers, to help patients discharge from rehabilitation and resume community living. Effectively communicate status of requested tasks to Clinical Care Managers both verbally and in writing on a regular and timely basis. Maintain thorough and pertinent records of services provided and information acquired. Monitor Discharge Bulletins and forward paperwork to Social Security to transfer patient applications to their home state/local office at discharge. Complete and submit applications for Long term Care Options. •This job description is not intended to be an exhaustive list of all duties.
