Job Description

Join Our Team at Valley-Wide Health Systems, Inc. Valley-Wide Health Systems is looking for a Community Support Specialist who is passionate about helping others and making a meaningful impact in the community. This role provides support, care coordination, and life skills assistance to help patients navigate challenges and improve overall well-being. Key Responsibilities Provide care coordination and life skills training for patients with significant mental illness individually and in group settings. Manage caseload for Community Support Services including the Permanent Supportive Housing Program. Develop, implement, and monitor individualized service plans. Coordinate care in clinic settings, patient homes, and community environments. Document patient interactions and maintain accurate clinical records. Ensure timely documentation (within 48 hours) and meet productivity and compliance standards. Collaborate with clinical teams to support patient care goals. Attend team meetings, individual supervision, and group trainings as scheduled. Demonstrate cultural competence and sensitivity in all interactions and documentation. Provide transportation using a company vehicle to patients in behavioral health services as necessary. Uphold HIPAA requirements and maintain strict confidentiality at all times. Requirements Bachelor's degree in Counseling, Psychology, Social Work or related field. Candidates with non-related degrees may be considered with one year of relevant experience. Preferred Qualifications Valid Colorado driver's license and ability to travel as needed. Ability to pass a Behavioral Health background investigation. Strong written and verbal communication skills. Proficient in Microsoft Office and general office equipment. Demonstrates empathy, professionalism, and cultural sensitivity. Excellent time management and organizational skills. BLS Certification (provided upon hire). Team-oriented with a collaborative mindset.
