Job Description
Position Summary Sirius Healthcare is seeking a compassionate and organized Care Coordinator to coordinate care for patients engaged through our community partner network, including substance use treatment centers, transitional housing programs, and nonprofit organizations. This field-based role is essential to ensuring patients remain connected to care by providing hands-on support, health education, appointment navigation, and follow-up services through both onsite visits and telephonic outreach. Key Responsibilities
Patient Engagement & Navigation:
- Meet with patients referred through community partners for intake, education, and care coordination
- Phlebotomy skills required
- Retain active Florida DOH HIV certification for testing and counseling is required
- Perform rapid HIV/HCV testing
- Complete state required forms DOH 1628 and
DOH 1818
as needed.- Complete case management encounter in Company EMR
- Assist with appointment scheduling, lab orders, and telehealth activation
- Conduct wellness checks and follow-up assessments to ensure continuity of care
- Address barriers to access including transportation, documentation, or medication adherence
- Participate in Company sponsored events and outreach is required and maybe during days, evenings and weekends.
Community Partnership Integration:
- Conduct regular onsite visits to assigned partner locations (e.g., sober living homes, IOPs, shelters)
- Collaborate with case managers, discharge planners, and staff to identify patient needs
- Serve as a liaison between community partners and Sirius Healthcare clinical teams
- Track referrals, touchpoints, and outcomes using company EMR
Documentation & Compliance:
- Complete and maintain accurate, timely documentation in the Company EMR
- Ensure adherence to
HIPAA, 340B
eligibility requirements, and care coordination protocols- Prepare reports and summaries as requested by supervisors or clinical leadership
Internal Collaboration:
- Work closely with Nurse Practitioners, Medical Assistants, and administrative staff
- Attend team meetings, case reviews, and training sessions
- Share feedback from the field to improve workflows and patient experience Qualifications
- 2+ years of experience in case management, social services, or community health
- Education minimum of high school diploma required.
- Phlebotomy Certification or minimum 1 year experience
- Maintain or Ability to retain BLS certification
- Experience working with underserved or high-risk populations
- Strong communication and interpersonal skills; ability to engage diverse communities
- Proficient in EMR and/or case management software
- Valid driver's license and reliable transportation required
- Ability to work independently and manage time effectively
- Bilingual in Spanish or Haitian Creole strongly preferred Preferred Background
- Familiarity with HIV, Hepatitis C, substance use, or mental health conditions
- Knowledge of local community resources.
- Experience working in residential programs, recovery housing, or health outreach settings Compensation & Benefits
Hourly Rate:
$19.65 - $22.00 (commensurate with experience and language skills)Mileage Reimbursement:
Paid for onsite visits except standard home base approved by the company at current standard IRS ratePerformance Bonus:
Up to 5% of annual salary, based on patient engagement and referral success metricsWork Schedule:
Full-time, Monday-Friday, with flexibility which may include evenings and weekends based on partner site needsPaid Time Off:
- 10 PTO days annually
- 1 personal day
- 9 paid holidays
