Full-time Functions:
Directly supervises the Care Coordination Team (CCT) in accordance with the Broward Behavioral Health Coalition (BBHC) contract, AHCA Targeted Case Management Handbook and Florida Certification Board, 65D-30, and agency standards. Provides direction and supervision to staff to ensure assessment, planning, linking, monitoring, coordinating, plan implementation, and advocacy for target population. Provides training, orientation, supervision, and evaluation of employee effectiveness of those supervised. Recommends hiring and termination of employees. Provides trainings to department staff. Meets with staff throughout the day to monitor and evaluate the performance of the staff in the coordination and delivery of services, and completion/coordination of admissions-assessments and intakes. Develops and monitors staff documentation deadlines and reviews staff's case management case notes, court reports, transition/discharge packets, treatment/service plans and reviews, updated plans, and amended plans for quality and compliance with agency standards. Maintains a caseload in the absence of case manager(s) and covers other caseloads, as needed. Schedules staff to ensure proper coverage and availability. Processes client assignments and other data into the EHR. Enters needed documents and information in the BBHC and Carisk portals. Provides services to persons served in his/her natural environment on a regular and emergency basis, as applicable. Completes utilization management applications, Level of Care Utilization System (LOCUS) assessments for enrollment and required documentation to BBHC and Carisk for approval. Provides supervision and direction to all team members. Participates in required agency community meetings as assigned. Completes and/or supervise the completion of agency reports or special projects, as assigned.
Requirements:
1. Prefer LCSW or LMHC. A master's degree from an accredited university or college with a major in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education or a related human service field and three years of full time or equivalent professional experience serving the target population. Or Have a bachelor's degree from an accredited university or college with a major in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education, or a related human services field (a related human services field is one in which major course work includes the study of human behavior and development) and have a minimum of five years of full time or equivalent case management experience serving the target population. And Bring to the position a certification in behavioral health case management supervision (CBHCMS) or have at least three years of experience in mental health with a master's degree, or five years of experience in mental health with a bachelor's degree. 2. Prefer experience working with the Broward County court system. 3. Familiarity with mental health and substance abuse system of care in Broward County. 4. Must be able to use a telephone, computer, copy/fax, and any other needed office equipment. 5. Required to acquire and maintain jail clearance. 6. Must have excellent organizational, time management, and writing skills. 7. Prefer past supervisory experience. 8. Required to cover other caseloads. 9. Must have strong interpersonal and communication skills. 10. Must have knowledge of the effects of substance use, mental illness and special population (medical issues, conditional release, personality disorders, etc). 11. Must possess a valid Florida Driver's License and have dependable transportation and qualified to drive agency vehicles. 12. Required to travel throughout Broward County for home visits, court appearances, and appointments . There may be some travel outside of Broward County. Supervise staff in the field. 13. Must apply for certification or be a Certified Behavioral Health Case Management Supervisor and maintain certification by complying with all necessary requirements as outlined by the Florida Certification Board (FCB). 14. Have knowledge of available resource in the community for adults with a serious mental illness. 15. knowledgeable to comply with state and federal statutes, rules, and policies that effect the target population.
Pay:
$45,000.00 - $50,000.00 per hour
