Provide clinical and psychosocial services to Acute Care patients by coordinating and facilitating the development of care plans. Provides through and accurate discharge planning information. Additional Requirements:

Successful pre-employment medical screening (drug test & TB) Level 2 Background Screening (DCF & AHCA) Completion of one week orientation Requires a Bachelor's degree in a human services field from an accredited college or university in social work, psychology, counseling or related field and 1 year of experience providing direct care services in a behavioral health setting. OR a Master's degree in a Human Services field; experience providing direct care services in a behavioral health setting preferred. Co-occurring experience preferred. Must be able to attain competency with the electronic health record and use it effectively within required timeframes.
