Job Description

Job Overview We are seeking a dedicated and highly skilled Renal Care Coordinator to join our healthcare team. The ideal candidate will possess extensive experience in acute and chronic kidney care, with a strong background in hospital and outpatient settings. As a Renal Care Coordinator, you will play a vital role in managing patient care plans, coordinating multidisciplinary services, and ensuring optimal treatment outcomes for individuals with renal conditions. Your expertise in critical care, trauma, and infectious disease management will be essential to providing comprehensive support to our patients. This role offers an exciting opportunity to make a meaningful difference in the lives of patients requiring specialized renal care. Duties Coordinate patient care plans for individuals with renal disease, ensuring seamless communication among healthcare providers across inpatient and outpatient settings. Conduct thorough patient assessments, including vital signs, medical history review, and physiological evaluations related to kidney function. Facilitate discharge planning and post-acute care arrangements, including referrals to nephrology clinics or home health services. Collaborate with multidisciplinary teams such as ICU staff, emergency room personnel, and behavioral health specialists to deliver integrated patient care. Manage documentation within medical records systems while adhering to HIPAA regulations to ensure patient privacy and data security. Assist with diagnostic evaluations and coordinate utilization management efforts to optimize treatment pathways and resource use. Provide patient education on renal health, medication adherence, lifestyle modifications, and disease management strategies. Support case management activities by monitoring patient progress, adjusting care plans as needed, and facilitating communication between primary care providers and specialists. Skills Extensive experience in critical care environments such as
ICU, PICU
(Pediatric Intensive Care Unit), or Level I Trauma Centers. Strong background in hospital medicine, emergency medicine, medical-surgical units, and post-acute care settings. Knowledge of physiology related to renal function, infectious disease care, Geriatrics, behavioral health, and emergency room protocols. Proficiency in medical terminology, diagnostic evaluation procedures, and medical records management. Familiarity with discharge planning processes, intake procedures, and outpatient clinic operations. Experience with managed care programs and utilization management strategies to ensure appropriate resource allocation. Ability to perform comprehensive patient assessments including vital signs and clinical observations within acute or inpatient environments. Excellent communication skills for effective patient service delivery and interdisciplinary collaboration. Demonstrated ability to navigate HIPAA compliance standards while managing sensitive health information efficiently. Pay:
$70,000.00 - $90,000.00 per year
