POSITION SUMMARY
The Care Coordinator is responsible for supporting patients and families by coordinating healthcare services, ensuring smooth transitions of care, and promoting adherence to treatment plans. This role involves working closely with providers, payers, and community resources to deliver patient-centered care.
ESSENTIAL FUNCTIONS
Care Management Monitor patient progress and address barriers to care. Facilitate communication between patients, clinical teams, and external partners. Documentation & Compliance Maintain accurate records in care management systems and EHR. Ensure compliance with organizational policies and regulatory requirements. Patient Support & Navigation Assist patient's parents in understanding their care plans and available resources. Coordinate appointments, referrals, and follow-up care across multiple providers. Resource Coordination Connect patients with community services, financial assistance, and support programs. Educate patients on preventive care and wellness initiatives.
KNOWLEDGE, SKILLS & ABILITIES
Strong communication and interpersonal skills. Ability to multitask and manage time effectively. Proficiency in care management software and electronic health records (EHR).

Empathy and patient advocacy, organizational skills, collaboration and teamwork, and problem solving.
EDUCATION & EXPERIENCE
Bachelor's degree in nursing, Licensed Practical Nurse certified, or a related field is preferred. Minimum of 1-2 years of experience in care coordination or case management and or patient services preferred.
SUPERVISORY RESPONSIBILITIES
Direct supervision of Care Coordination staff.
WORKING CONDITIONS
General office working conditions. May require occasional overtime during peak periods, such as month-end or year-end closings.
