Job Description

Job Overview The Care Coordinator plays a vital role in ensuring that patients receive comprehensive and compassionate care. This position involves collaborating with healthcare professionals, patients, and families to develop and implement effective care plans. The Care Coordinator will be responsible for managing case details, monitoring patient progress, and providing direct support to enhance the quality of life for individuals in various care settings, including memory care and hospice care. Duties Develop and implement individualized care plans based on patient assessments and needs. Monitor and evaluate patient progress, adjusting care plans as necessary. Conduct vital signs assessments and maintain accurate health records in compliance with HIPAA regulations. Provide direct support to patients, assisting with daily activities and ensuring their comfort. Collaborate with nurses, physicians, and other healthcare professionals to coordinate comprehensive care. Offer first aid as needed and ensure that all safety protocols are followed. Engage with families to provide updates on patient status and involve them in the care process. Assist in case management tasks, including documentation and reporting of patient outcomes. Experience Previous experience in a healthcare setting is preferred, particularly in roles involving case management or direct patient support. Familiarity with memory care or hospice care practices is a plus. Strong understanding of HIPAA regulations to maintain patient confidentiality. Ability to lift patients or assist them with mobility as required. Certification in first aid or CPR is desirable. Excellent communication skills to effectively interact with patients, families, and healthcare teams.

$17.00 - $20.78 per hour
