The Elder Care Coordinator supports clients and families through the healthcare, legal, and long-term care landscape — serving as a compassionate liaison between clients, families, healthcare providers, and care facilities to ensure continuity of care aligned with the firm's legal strategies. Responsibilities What You'll Do
Conduct client assessments and develop care plans aligned with client goals and legal payment strategies
Research facilities, coordinate admissions, and assist families through transitions of care
Advocate for clients with providers and facilities, and help resolve care or billing issues
Obtain medical documentation for Medicaid, VA, and other public benefits programs
Build and maintain referral relationships with long-term care facilities, hospitals, and healthcare professionals
Educate community partners on the firm's elder law and care navigation services
Collaborate with the marketing team to expand the firm's community presence
