Description Care Coordinator (On-Site) We are seeking a compassionate and detail-oriented Care Coordinator to support individuals by providing information, referrals, and guidance in a fast-paced, service-driven environment. This role requires strong communication skills and the ability to remain calm and effective in high-stress situations. + Provide information and referral services to clients + Support individuals in crisis or high-stress situations with professionalism and empathy + Coordinate services and connect clients with appropriate resources + Maintain accurate documentation and case records + Communicate effectively with diverse populations and internal teams

Requirements Qualifications:

+ Bachelor's degree in Social Work, Psychology, or related field required + Experience providing information and referral services required + Minimum 1 year of experience in crisis counseling or clinical work required + Experience working with veteran populations preferred + Strong interpersonal and communication skills + Ability to manage multiple tasks in a fast-paced environment + Proficient in Microsoft Office and general computer systems + Bilingual (English/Spanish)
