We are currently seeking a dynamic and detail-oriented individual for the position of Patient Care Coordinator in our Donor/Third Party IVF department. Duties Include but not limited to: • Case Management of "third party" IVF cycles Coordinating patient care plans by liaising with all necessary care providers and nurses to ensure patient needs for prescreening Collect and maintain health history of patient, maintain medical records for cycle clearance Ensure consent forms and other cycle related documents are completed and archived Prepare patient for exams and procedures Assist in patient calls and authorize prescriptions as approved by physician Monitor patient's progress during IVF cycle and follow up on any missing items Work to create efficient systems and workflows within the department Other duties as assigned Job description and responsibilities subject to change
Skills and Qualifications:
Minimum of 2 years working in a clinical or social work setting Excellent computer skills Familiarity with electronic medical records software preferred Strong organizational and time management skills High Level of customer service Attention to detail and independent
Education:
Minimum of High School degree required. College degree strongly preferred.
Work Schedule :
Monday through Friday with weekends as needed/scheduled

$22.00 - $25.00 per hour
