St. Augustine Youth Services (SAYS) $22 hourly additional bonuses are available Are you passionate about helping youth and families thrive? St. Augustine Youth Services (SAYS) is seeking a dedicated and experienced COACHES Care Coordinator to support children with mental and behavioral health challenges through individualized, community-based services.
What You'll Do:
As a COACHES Care Coordinator, you will: Serve as a targeted case manager, ensuring consistent support and coordination of care for youth and families. Facilitate wraparound meetings and coordinate services every 60-90 days. Support transitions from acute and restrictive settings to community-based care. Reduce hospitalizations, arrests, and out-of-home placements through proactive case management. Connect families to essential services and resources with a focus on wellness and recovery. Maintain accurate documentation, meet contract standards, and uphold confidentiality. Transport clients to appointments and school as needed. Participate in referral interviews, team meetings, and community events. Support agency fundraising efforts and perform other duties as assigned.
What We're Looking For:
