Job Description

Job Overview We are seeking a proactive and detail-oriented Referral Coordinator to join our dynamic healthcare team. In this vital role, you will serve as the primary point of contact for managing patient referrals, ensuring seamless communication between patients, providers, external partners. Your expertise will facilitate efficient scheduling and coordinate necessary documentation to support optimal patient care. HIPAA (Health Information Portability and Accountability Act)If, in the normal course of my duties and responsibilities, I am required to access protected health information (PHI) and electronic protected health information (EPHI) for the purposes of treatment, payment and operations within Good News Clinics, I will limit such access to only the minimum necessary amount of PHI and EPHI necessary to perform the functions of my job. If access is not required in the normal course of my duties and responsibilities, I will not access PHI or EPHI. Job Specific and Unique Knowledge, Skills and Abilities 1.Manage external referrals queue in EPIC and maximize patient load agreements with outside offices/facilities. 2.Update patient records as applicable, verify patient information as accurate before completing referral. 3.Knowledgeable of patient care process and offices the patients are being referred to. Essential Tasks and Responsibilities 1.Coordinate with other Good News Clinics (GNC) departments to ensure patient customer service 1A Receive, log, and review requests for specialty care 1B Announce changes in care availability at morning huddles 1C Attend and brief monthly management and team meetings 2. Facilitate collaboration with community partners 2A Work closely with all medical practices who volunteer with Good News Clinics 2B Ensure that each practice contact person understands GNC procedures and processes 2C Reach out to new hires at these outside facilities and offer information 2D Work closely with Northeast Georgia Medical Center staff 2E Coordinate services needed by GNC patients; Become the contact person for case management for coordination of services 3.Compile monthly statistical reports regarding patient care activities and services provided 3A Maintain patient tracking database and update records as needed 3B Provide reports to the Department of Public Health, Volunteers in Medicine Program, and GNC administration 4.Facilitate collaboration and demonstrate problem solving skills 4A Liaison with providers to understand a patient's need and a medical office's ability 4B Bring errors and process inefficiencies to GNC leadership's attention 5.Ensure that patient referrals are processed in a timely manner 5A Review referrals to determine that practice agreements are honored 5B Ensure appropriate medical work-up and diagnostic testing is completed prior to scheduling any specialty care appointment 5C Prioritize referrals according to urgency, utilize physician input where appropriate Join us in delivering exceptional healthcare coordination that enhances patient experiences!
