A&C Private Home Care has been a trusted provider of home health services to medically fragile adults and children since 2015. Located in Norcross, GA, we are a growing home care agency that takes a revolutionary approach to meeting our clients' needs-one client at a time. Our dedicated team includes compassionate caregivers, Care Coordinators, and professional clinical nurses, all working together to deliver high-quality care.
Position Overview:
We are currently seeking a dynamic Home Healthcare Care Coordinator who excels in verbal and interpersonal communication skills. In this role, you will interact daily with clients, client representatives, and caregivers. You will be responsible for a variety of tasks that ensure smooth operations and quality care for our clients.
Key Responsibilities:
Screen, hire, and onboard new caregiversSchedule caregivers with clients and organize client visitsDocument and resolve issues reported by clients, caregivers, and case managersConduct daily check-ins with clientsEnsure caregivers fully understand their expectations and responsibilitiesReview and reinforce company policies and procedures with caregiversRotate on-call weekends with a designated day off during the weekProvide direct home care services when a caregiver is unavailable
Ideal Candidate:
The ideal candidate will be organized, detail-oriented, and capable of working efficiently in a fast-paced environment while managing multiple important tasks. You should be able to collaborate effectively within a team, demonstrate punctuality, and maintain patience and composure under pressure. High-quality work production and attention to detail are essential in this role.
Job Requirements:
Current CPR/First Aid certification and a recent TB Skin TestFingerprint background check (administered by FieldPrint)Proficiency in Microsoft Office and general computer knowledgePrevious experience as a caregiverFamiliarity with Medicaid Elderly and Disabled Waiver Program (EDWP), Independent Care Waiver Program (ICWP), and Georgia Pediatric Program (GAPP)
