Early Intervention Care Coordinator

The Parent Child Development Center Ewa Beach is seeking to hire a full-time Care Coordinator to coordinate and provide services for infants and toddlers with developmental delay(s) in Ewa Beach communities. This is a case management role with opportunities for utilizing clinical skills, in accordance with appropriate credentialling. Job responsibilities include, but are not limited to: Meeting with families, providing information on the program, conducting assessments (intake, routine based assessments), coordinating home visits and meetings, developing Individual Family Support Plans and outcomes for the child's development, timely documentation, and connecting families to needed resources.

Requirements:

A valid drivers license, reliable transportation and at minimum a Bachelors Degree in Social Work, Human Services, Psychology or related field. Social Work background is highly desired. Salary offers commensurate with education, licensure and experience.

Pay:

$45,000.00 - $52,000.00 per year
