Job Summary and Responsibilities Coordinates care with other health care community providers. Promote patient retention by providing focus on getting patients to the right service with the right provider at the right time - one call resolution. Manage phone call activity to minimize any delays experienced by customers. Conduct intake for new referrals to quickly and accurately triage the needs of the caller. Job Requirements Preferred Bachelors degree in Social Work or Human Services related field Acute care experience in social work or behavioral services
