Compensation will be discussed with the hiring manager and is based on licensure, certifications, qualifications, and relevant experience. Summary of Responsibilities At our Secure Residential Treatment Facility, we provide compassionate, high-quality care for adults living with severe and persistent mental illness, all within a collaborative team environment. The Care Coordinator / Discharge Planner plays a vital role in guiding safe, clinically appropriate transitions of care by coordinating individualized discharge planning, continuity of care, and community resource connections from admission through discharge. In this role, you'll collaborate closely with youth, families, interdisciplinary teams, and community partners to support successful outcomes and least restrictive care planning. Success requires strong clinical judgment, exceptional communication, critical-thinking skills, and the ability to navigate complex systems with empathy and professionalism. Qualifications Hold a bachelor's degree or higher with an emphasis in psychology, counseling, social work, or related field required. Complies with accepted professional standards and is licensed or license eligible for the state of Idaho, required. Experience with discharge planning, continuity of care coordination, family engagement, and community resource linkage strongly preferred. Three (3) years of experience working with adolescents with severe and persistent mental or behavioral health needs in a psychiatric, residential, inpatient, or care coordination setting preferred. Knowledge of PRTF regulations, Idaho YES System of Care principles, Medicaid, and community behavioral health resources preferred. Obtain and maintain current BLS CPR. Licensed driver with an automobile that is in good working order and ability to drive various weather conditions to ensure reliability. Headwaters Behavioral Health is an equal opportunity employer.
