Job Summary:
Community Care Program interns will complete comprehensive community-based assessments for older adults in their homes, educate participant's on appropriate resources and services in the area, and assist with program referrals and benefit applications as needed. Duties and Responsibilities as a
Certified Care Coordinator:
Complete the Illinois Department on Aging's CCP Care Coordinator Certification Training. Complete In-Home Needs-based Assessments. Develop a Plan of Care for participants that addresses needs identified during the assessment process. Refer participants to various local resources. Assist with benefits applications, including but not limited to Medicaid and SNAP. Create and maintain client files and documentation as needed and consistent with the Illinois Department on Aging and as determined by ECS Policy and Procedures. Maintain a strong working relationship with internal and external professionals utilizing them whenever appropriate on mutual or potentially mutual client cases. Responsible for attending all staff meetings and providing necessary reports when requested, and meeting or exceeding annual service projections. Keep abreast of current events in the aging field by attending appropriate professional development training, including those necessary to maintain current certification and state requirements. All other duties as assigned by the Supervisor or Management staff. Knowledge, Skills, and Abilities Ability to assess the needs of older adults and address program-related concerns. Ability to keep accurate and timely case files and notes. Excellent communication skills. Strict adherence to standard confidentiality codes. Must be able to travel to and safely enter homes and other locations to perform job-related duties outside of the office environment. Credentials and Experience A Bachelor's of Arts or Science in Health or Human Services, Social Work, or other related field. IL Department on Aging Care Coordinator Certification, or become certified within 90 days of hire. Access to a reliable automobile with a valid driver's license and insurance. Other Characteristics All Elder Care Services employees are expected to maintain a positive work environment by building rapport with participants, co-workers, and the community in a courteous, attentive, and professional manner. They must display a high degree of honesty, loyalty, and integrity, and assume full responsibilities for their duties and assignments. They must demonstrate the ownership and pride required to ensure productivity towards achieving the goals of Elder Care Services.
