Part-Time Medical Assistant / Care Coordinator (Gujarati Speaking)

Job Summary We are seeking a dedicated and compassionate Part-Time Medical Assistant / Care Coordinator who is fluent in Gujarati to join our healthcare team. This role offers an exciting opportunity to provide exceptional patient care and support via CCM phone calls. As a vital member of our team, you will assist with making Chronic Care Management (CCM) calls to patients . This role is phone-based and coordination-focused , not a traditional clinic MA position. The ideal candidate is comfortable speaking with elderly patients daily, building relationships, and staying organized with ongoing follow-ups. Your energetic approach and cultural sensitivity will help foster trust and comfort for our diverse patient population. Responsibilities:

Manage a panel of CCM patients (based on hours) Ensure every assigned patient is contacted monthly

Track:

Completed calls Patients needing follow-up Patients who are unreachable Conduct CCM Calls (Daily Responsibility): Make structured calls to patients in Gujarati and English , including: Medication review Symptom check General health status Identifying new issues or concerns Reinforcing care plans

Documentation & Time Tracking:

Document all calls accurately in the EMR Track time spent per patient to meet CCM requirements Maintain organized records of all interactions

Follow-Up & Persistence:

Re-attempt calls for patients who do not answer Follow up on: Patient concerns Care coordination needs Ensure no patient "falls through the cracks" CCM Enrollment (Secondary but Important): Contact eligible patients and explain the CCM program Enroll patients into CCM services Track enrollment status: Enrolled Declined Pending follow-up

Post-Home Health Follow-Up:

Identify patients recently discharged from home health Call within 3-7 days Transition patients into CCM when eligible

Care Coordination Support:

Communicate patient concerns to: Nurse Practitioners LPN Assist with: Scheduling follow-ups Coordinating labs/referrals

Performance Expectations:

This role is metrics-driven .

The candidate is expected to:

Complete all assigned monthly CCM calls Maintain high call completion rates Enroll new CCM patients regularly Keep accurate and organized records Communicate effectively with elderly patients

Qualifications:

Fluent in Gujarati and English Strong phone communication skills Comfortable speaking with elderly patients daily Organized and detail-oriented Reliable and consistent

Preferred:
