Social Service/Discharge Planner

We are leaders in the long-term care industry, supported by a dedicated team of experienced, energetic, and compassionate RNs, LPNs, and CNAs. We are currently seeking a motivated Social Services / Discharge Planner to join our growing team. In this role, you will support both resident social services and discharge planning needs.

Responsibilities:

Assisting with psychosocial assessments, addressing residents' emotional and social needs, and collaborating with interdisciplinary teams to ensure comprehensive care planning. Guiding residents and families through the discharge process, provide education on post-care needs, coordinate with healthcare staff and external agencies, and ensure smooth, compliant transitions of care. Accurate documentation and adherence to federal, state, and facility guidelines are essential.

$25.00 - $28.00 per hour
