Job Description

Lafayette Franciscan Healthy Living Clinic 1116 N 16th St Lafayette, Indiana 47904 The Home Care Coordinator is an important role within the Franciscan Health team. This position is responsible for the coordination and implementation of home care services. These home care services extend to post-acute program participants.

WHAT YOU CAN EXPECT
Practice Hours:
Monday
  • Friday, 8:00 a.m.
  • 5:00 p.
m. No Weekends, Evenings, or Holidays Arrange for home care, Durable Medical Equipment (DME) and supplies, as needed. Provide intake data and essential background information and coordinate orders, as indicated to members of the multidisciplinary team on request. Assess the homecare needs of post-acute participants, and participate in the development of specific plans of care. Communicate and collaborate with members of the IDT in the delivery of services to patients/families. Coordinate scheduling and education of outside vendor partners, specific to applicable post-acute requirements.
QUALIFICATIONS
High School Diploma/GED
  • Required Bachelor's Degree
  • Preferred 1 Year Working with Frail or Elderly
  • Required 2 Years Home Care
  • Preferred
TRAVEL IS REQUIRED
Up to 20%
