LifeSpring Health Systems is seeking a Certified Peer Support Specialist (CPSP) to support older adults (age 60+) living with serious mental illness (SMI) in rural communities. This position is part of a grant-funded initiative designed to improve access to behavioral health services, strengthen care coordination, and support older adults in maintaining independence within their communities. The Peer Support Specialist will deliver person-centered, community-based services that support engagement in care, navigation of health systems, and coordination across behavioral health, primary care, and aging services. This role is integrated within a broader care team and contributes to services that align with Medicare Part B behavioral health service models, including care coordination and peer-supported navigation. The position is grounded in the 4Ms Framework (What Matters, Medication, Mentation, Mobility) and emphasizes sustained engagement, continuity of care, and reduction of barriers to service access. Key Responsibilities Conduct outreach and engagement with older adults with SMI in home, community, and clinical settings to support connection to care; Build and maintain ongoing relationships that support adherence to recommended behavioral health and medical services Support individuals in identifying personal goals and priorities ("what matters") and aligning services accordingly; Provide care navigation and coordination services, including assistance accessing behavioral health, primary care, and aging-related services; Facilitate warm handoffs and follow-up to ensure successful linkage between providers and continuity of care; Collaborate with clinical and interdisciplinary teams to support implementation of care plans and service utilization; Assist individuals in understanding and accessing Medicare-covered behavioral health services, including support with appointment follow-through and engagement; Address social and structural barriers to care, including transportation, cost concerns, and system navigation; Support functional independence through connection to services addressing activities of daily living (ADLs) and community participation; Document all activities in accordance with organizational, regulatory, and payer requirements to support service delivery and reporting; Participate in team meetings, case reviews, and care coordination activities to ensure integrated service delivery; Conduct outreach and maintain partnerships with community-based organizations to support referral pathways and service access. Service Delivery Model Alignment This role contributes to service delivery models that include: Community-based care coordination and health system navigation Integration of behavioral health, physical health, and aging services Team-based care planning and implementation Support for Medicare Part B behavioral health services, including care coordination and peer support components Ongoing engagement and follow-up to support service utilization and outcomes All services are delivered in coordination with licensed providers and within established care plans to support medical and behavioral health needs. Approach to Care This role is grounded in: Person-centered, strengths-based engagement Respect for lived experience and individual autonomy Relationship-based support that promotes trust and sustained connection to care Meeting individuals in community settings to reduce barriers and improve access Supporting stability, independence, and quality of life for older adults Minimum Qualifications Certified Peer Support Professional (CPSP) in Indiana, or eligible and willing to complete certification within a specified timeframe as a condition of employment To be eligible for CPSP certification, candidates must: Be at least 18 years of age Have lived experience with a mental health condition OR lived experience supporting a close family member or someone with a mental health condition, and demonstrate the ability to use that experience to support others in a peer-based, person-centered role Have a high school diploma or equivalent Be willing to complete required state-approved training and supervision hours Demonstrated ability to engage older adults with complex behavioral health and medical needs Strong communication and interpersonal skills Ability to work independently in community-based settings Valid driver's license and reliable transportation Preferred Qualifications Experience working with older adults (60+) and/or individuals with SMI Experience in rural or underserved communities Familiarity with care coordination, health navigation, or integrated care models Experience working within or alongside Medicare-funded services Knowledge of local healthcare, aging, and social service systems Why This Role Matters Older adults living with SMI in rural communities face significant barriers to accessing coordinated, effective care.
