We are seeking a dedicated, compassionate, and highly organized Homecare Coordinator to join our team at [Company Name]. This role will be responsible for managing and coordinating home care services for patients, ensuring the delivery of high-quality care, and handling various office administrative tasks to ensure smooth daily operations. The ideal candidate will work closely with caregivers, healthcare professionals, patients, and their families to provide excellent care and keep all aspects of the office running efficiently.
Job Duties and Responsibilities:
Patient Care Coordination :
Coordinate Patient Care :
Assess patient needs and develop personalized care plans in collaboration with patients, families, and medical professionals.
Schedule and Assign Caregivers :
Manage daily caregiver schedules, ensuring proper staffing based on patient needs and care plans.
Monitor and Supervise Caregivers :
Oversee caregiver performance, provide feedback, training, and support as necessary.
Communication and Follow-Up :
Maintain regular communication with patients, families, and healthcare providers to ensure high-quality care and continuity.
Problem-Solving :
Address issues related to patient care or staffing, ensuring resolutions that maintain patient satisfaction.
Ensure Compliance :
Follow all company policies, healthcare regulations, and local laws. Keep up-to-date patient records and documentation.
Office Administration :
Office Management :
Handle general office administrative duties, including answering phone calls, responding to emails, and greeting visitors.
Documentation and Record Keeping :
Maintain accurate records of patient care, caregiver assignments, and office communications. Ensure proper documentation of all services rendered.
Billing and Insurance :
Assist with client intake, verify insurance, manage billing and payments, and ensure that all documentation is accurate and up-to-date.
Scheduling :
Coordinate office and staff schedules, ensuring all meetings, appointments, and caregiver shifts are properly organized.
Inventory Management :
Monitor and order office supplies, caregiving materials, and equipment as needed.
Data Entry and Reporting :
Input patient and caregiver data into the company's systems, generate reports, and track service delivery metrics.
Qualifications :
Education :
High school diploma or equivalent required. Associate's or Bachelor's degree in healthcare management, office administration, nursing, or a related field preferred.
Skills :
Strong communication and interpersonal skills. Excellent organizational, multitasking, and time management abilities. Ability to manage office operations and maintain a positive and efficient environment. Knowledge of home care regulations and standards. Proficiency in office software (Microsoft Office, scheduling tools, and/or healthcare management software). Basic understanding of medical billing, insurance verification, and office administration processes.
